We are willing to accept assignment (direct payment from your insurance company) as a courtesy to our patients. Although, as dental plans have become increasingly complicated and reimbursement levels are unpredictable, we must limit our payment terms to the following two options. Please let us know at your next appointment which option you prefer:

Option 1: Easy Pay: We will continue to accept the assignment of your insurance benefits as long as we have
signed consent that authorizes us to debit your credit card for any remaining balance after we receive payment from your insurance company.

Option 2: Non-Assignment: Pay for your dental treatment in full on the day of your appointment by way of Visa, MasterCard, debit or cash. If your insurer is online with CDAnet, we can submit your dental insurance forms electronically on the day of your visit, and you should be reimbursed within 3 – 10 days.

Please remember that you are fully responsible for all fees this office charges regardless of your insurance coverage.

Please note: If you are sending your children by themselves to their appointments, please make sure prior payment arrangements have been made.

This warranty will be valid provided you:

• Practice proper home care (brushing twice daily and flossing once daily)

• Have professional cleanings as recommended by our dental team (at least twice per year)

• Agree to wear any protective night guard when recommended

Your dental plan is a form of compensation provided by your employer. You can expect the carrier (insurance company) to reimburse you for a portion of our fee. Although we at Spencer Dentistry are not party to the contractual arrangement with your insurance company, we do want to help you receive the maximum reimbursement to which you are entitled. As a convenience to you, we will help you process your insurance claims in order for you to receive the maximum benefit from your insurance company. We will also gladly provide dental x-rays and a written diagnostic report should your insurance company have any questions about the services provided.

You can be confident that we will always provide you with the best services at all times, regardless of the limitations imposed by your insurance coverage. To do otherwise would violate our contract with you — a contract we feel morally obliged to honour.
